    Hmmm ... they dont normally keep the form. They have always returned it to me along with the receipt. I think you might need the form later on when you visit MOFA if they approve the visa (or maybe not i applied manually through istiqdam and they took the online generated signed and stamped form at MOFA). However just wait it out and see what the result shows up online.
