There really is no BEST insurance. There are a large number of international Insurance Companies working in Spain:
Allianz, Zurich, Groupama, American Life, etc.
Solid home-grown ones:
Catalana-Occident, Mapfre, etc.However, I would suggest you speak to an insurance broker. Given the very wide coverage you appear to need, he will advise you on the best for each risk. A broker also has the advantage in that, if you have several companies, you have a single point of contact for any claims you may have to make. A broker is also probably going to be a little more "on your side" as he wishes to retain your business.

As for me, I like Globelink travel insurance. It is an UK based company that have an office in Spain but it can easily be found on the website www.globelink.co.uk

There really is no best insurance company - your needs really dictate which company you should consider using.
A good broker will recommend companies and policies that best match your needs, they'll have experience working with the various companies and know the service levels, limitations etc.
Cheapest isn't always best and having a good intermediary on your side when you have a problem or need to claim is extreemely useful and for many expats I'd actually say almost essential.
