Visa and Iquama Name differ from Passport Name..

Hello guys, me myself Bijay from Nepal, Currently my Father is enjoying his holiday in Nepal. But today he got in big problem..
In his Visa and Iquama his name is "Salyan Rai" and in his Passport and Citizenship his name is "Krishna Rai". Now today Nepal Government Official rejects his visa due to this difference in name...
So, please Anyone guys if you know how to change name in visa and iquama, then please suggest me..
