    Brother, as per my past experience I can tell you the following:
    1 do not write visitors name in Arabic, write as on passport.
    2. Write more than two words in name. If name has only two name suffix father/husband name.
    3. I think medical is not a good reason- write to visit her husband
    4. Must write city in your address
    5. Apply from the city your iqama is issued
