    In رقم السجل column you should write your company CR(Chamber Record) number, and if your sponsor is an individual(Kafil) then you should write his chamber number.

    In Kaqam Tashira Daqool column, you should write your visa number printed in your passport when you first entered KSA.

    And you shall no where in the application form write your Kafil ID number. Kafils ID is his national id, and it has nothing to so with your visit visa application.
    The visitors will be upon your sponsorship not your Kafil so, why would you write his ID number.
