2 photocopies of the ID.
Original and photocopy of the High School Diploma (Bachillerato Educacion Media).
Original criminal record (hoja de delincuencia).
SINPE bank account number (17 digits) in colones under your name.
Foreigners should bring the entire above plus:
Photocopy of passport.
Original and photocopy of valid Residency ID.
Original and photocopy of the CCSS ID (this should be the Social Security slip from prior employer, the copy of the receipt provided by CCSS, or the Social Security ID).
