The requirement are:
1. Passport copy of the Husband (Writie the same name as in passport if capital than capital)
2. Your Iqama (Writie name as per Iqama)
3. Fill the form and practice (As many you want to writie , no problem)
4. When you thin you have done correct (Only the husband name will come as English if it is in English in passport) . all other Arabic
5. Get print
6. Your sign
7. go to your Jawazat (Stamp)
8. if govt employee like university professor Go to MOFA directly
9. if not Govt employee than (Ask some body)
